TeodorKOZAK94, passed away peacefully at Avon Lodge Care Home, Bristol on the 8th October 2018. The beloved husband, father, grandad and great grandad will be greatly missed by his wife Rosaria, his son Peter, his daughter-in-law Tracey, his three grand children, Hannah, Joseph and Laura; and his great grand daughter, Emilia; also a special recognition to Natalie and Carl. The funeral service will be held at St. Joseph's Catholic Church, Forest Road, Fishponds, Bristol BS16 3QT at 10.45 am on Friday 9th November 2018. Followed by the burial at South Bristol Cemetery and Crematorium, Bridgwater Road, Bristol BS137AS. Please make any charitable donations to Dementia UK.
