Died peacefully at Yeovil District Hospital on the 4th December aged 91 years.
A beloved Mum, Nan and Great Nan.
She will be greatly missed by all her family and friends.
Funeral service takes place at St. Bartholomew's Church, Crewkerne on Thursday 14th December at 11am.
Donations in memory of Phyllis are invited in aid of Dementia UK and may be left after the service,
